Primary Care Network

Leeds Student Medical Practice is working closely with the Light Surgery as part of a Primary Care Network.

What is a Primary Care Network?

PCNs are essentially GP practices joining together to work locally with community service providers and other health and care organisations.

The concept of Primary Care Networks (PCN) was first introduced in the NHS England long term plan when it was published at the start of 2019.

PCN’s became formally established in July 2019 with an aim to dissolve the divide between primary and community health services.

Primary Care Networks consist of general practices in a locality working collaboratively to deliver enhanced services for the whole patient population.
